Ugly and perhaps a little confusing... but if it could be done in a less
ugly manner then it could be more useful. Perhaps not having the empty
tick box but only the ticks?

I concur with this comment. The boxes are uncalled for. I also think that
the check marks are sort of odd to use in such a fashion. I could see it
for a multi page story or equivalent.
