user defined download page

Senario: A user wants to dowload a free whitepaper from my site. But i want to
know who is downloading what. So they are taken to a form to fill out name
company and phone #. They also select which whitepapers they want, there will
be about 10 to choose from. Once they submit that info they are taken to a
download page where the previously selected whitepapers are ready for download,
either manually or automatically. They get the file and I get an email with
what they are downloading. What's the best way to do this. Can url variables
handle this or do I need to build a small shopping cart system? Any tips will
help.
