Survey Buttons

I have a series of images on my website, and I want to set up something that
will allow people to rate the images. It could be as simple as a radio button
that says "Good" under each one. Can someone point me towards a tutorial where
I can have the button for each image that would submit their opinions to me?

You could look at www.hotscripts.com
I'm sure they'll have some type of 'rating' script that you can use...
depending on your server model.
