Chris -- You might want to just read lots of these notices on other sites,
assess which statements and clauses are appropriate for your use, and write
up your own. A higher-budget option would be to consult a lawyer.

I don't know of any templates or forms for those kinds of documents - I
don't have them in my Tools package (see my signature below) but maybe I'll
think about adding some in the next version.
