Problems connecting to database

Hey, I am trying to connect dreamweaver to my php/mysql database on my
website... I have tried setting up everything in the testing server interface
for the site and then made a new connectiion under application with all the
ifno for the database... however...

when I click test, it gives me the following error:

What do I do?
