Placing text with forms

Hey, this may be a silly question but I would feel silly if I don't ask.

I trying to align my text and form boxes, so that my text is aligned, and my
boxes are all aligned evenly. However I can't seem to have any control of the
text and text box. It ony lets me type one space and then I have to enter the
box. Any suggestions?

Use a table and style as needed.
First column your text, 2nd column your text boxes.
