Library Items and Templates Not Updating

I am using Dreamweaver MX 2004. We recently did a site overhaul, and during
that process, I noticed that when I updated a template and then responded "yes"
to the "update all files using this template" prompt, only some of the files
using the template were updated. I am working now on updating a library item
that is used site-wide. I've made the change, responded that the files should
be updated, and while I get confirmation that the files have been updated, when
I pull one up to check it, it has not, in fact, been updated. Even when I do
"update this page," the library item is not changed.

Any suggestions would be welcome.

Here are five ways to make changes not propagate from Template to child -

1. Rename the Templates folder
2. Move the Templates folder to some other folder level in the site other
than root level
3. Move a template out of the Templates folder.
4. Make changes to an editable region of the template (which will not
propagate)
5. Corrupt the site cache so that the link between template and child page
is broken

The latter problem can be repaired by using SITE | Advanced > Recreate Site
Cache, and the three others can be fixed by just not doing them. Have you
done any of those?

I have not done the first four, and I have recreated the cache more than once. The problem persists.

How large is the site?

Try deleting the site definition, and then recreating it. Do updates work
now?
