Issues with css/php/ and ? require_once

I have a page http://www.clubwizard.co.uk/reviews/gallery.php
i have used the require_once command a few times (the main menu and
quick guestlist menu's on the left) i have also tried to use it on the
righthand side for the top 10 guestlist venues section - the include is
there however it will not load
