form verify works in preview but not on site ?

At this page : http://www.4thstar.net/whois.html
i have a form verification that pops up a message box saying field is required , this is to replace the old screen you may see .

the problem is that i have the script on the page , so when i do a preview from Dreamweaver the script runs fine and i get the popup, but once i have uploaded the page it still goes past the check and loads the old screen..
whats strange is that i have used same behaviour on another page and that one works fine ?

any suggestions

1) Are you sure you get the new page and not a page that's cached in
your browser or at your ISP?
2) Can you give a URL?

I get directed to a page that says "Your Domain Name cannot be less than 3
characters". However, before that I get an error message that says

Could you be seeing a cached version of the page?

i am sure i cleared the cache, but still get the old page and not the new popup ?
strange

the errors however are a little worrying to me as i have not yet seen these
