form help pls

If anyone has the time, I am stumped why one of my forms won't work.

I reduced all the required fields down to one to help me troubleshoot. The hidden fields I left the same as my contact page (which works) to also help me narrow down the problem.

The only thing different about this form and the contact page is the use of a nested table in the forms heading.
