form creation tool or help :)

Hello.
I guess first i am looking for a program/add-on for dreamweaver4 , or a stand alone program that allows creation of forms , i want to validate some forms before they are processes.

i have found a few in the extensions but they are great for simple checks but i need a little more complex checks..

sorry for the format , i know visual basic , but not java , so i guess those of you who know java will know what i am trying to do

1: if hidden field = "co.uk" then only display 2,4,6,8,10 in the list/menu dropdown

This can be done in Javascript, but I would need to know two things:
(1) How does "co.uk" get written to the hidden field?
(2) What event would trigger a change in list/menu?
