Fill in form before proceeding to download

I need some help or a script that will prompt the user to fill in and submit a form before proceeding to a download of a PDF

The script would be javascript
I would like to have a name and email validation
and yes just 1 file to download
I have had a look on hotscripts.com but are not sure that I am getting the
right script
any suggestions?

Which cgi form handler are you using?

If it allows you to create your own validation page you could add the link to
the pdf file to that, without having to add any further scripts.
