Defining a Database Connection

I'm trying to define a database connection but I have no clue where to
begin. I have installed XAMPP which bundles MySQL, PHP, Apache, etc all in
one. Through Dreamweaver I have tried everything possible to connect
through the MySQL Connection dialogue box. Here is where I am at in this

Can someone please give me the exacty information I need to enter into each
of the fields of that box? I am totally new to PHP/MySQL!
