checkbox or drop down menu

I need to add a form that will allow customers to choose different items to add
to an order.
Example:
They are ordering a necklace. They need to choose color and type of jewel or
pearl to use in the custom necklace.
I am not a programmer so not sure how to do this in Dreamweaver CS3. I would
like to find a simple program that would let me build this wysiwyg.
